NTSB releases preliminary report on UPS plane crash

A man who was critically injured in a fiery crash of a UPS cargo plane in Louisville, Kentucky, has died more than 7 weeks later, bringing the death toll from the crash to 15.
Louisville Mayor Craig Greenberg identified the deceased as Alain Rodriguez Colina, and said he died Thursday after being seriously injured in the crash.
“It is with great sadness that I just learned that Alain Rodriguez Colina has passed,” Greenberg wrote in an X post Thursday evening. “Alain is the 15th victim of the crash of UPS Flight 2976. He was seriously injured during the crash and passed away earlier this Christmas day. May Alain’s memory be a blessing.”

Kentucky Gov. Andy Beshear responded with the updated death toll from X, urging people to “pray for these families today and in the days, months and years to come so they know they are not alone and they are loved.”
Colina’s death marks the latest development in one of the deadliest aviation tragedies in the US this year.
On November 4, UPS Flight 2976 crashed seconds after takeoff from Louisville International Airport, crashing into a nearby industrial complex, killing three crew members and 11 people on the ground.
The first report of the National Transportation Safety Board (NTSB) said that the left engine of the cargo plane bound for Honolulu was separated from the plane. and burst into flames shortly after takeoff.

The black box data shows that the plane only came 30 meters above ground level before crashing.
Airport surveillance footage shows the left engine and propeller separating from the wing shortly after the plane went around, with the left engine on fire, according to the NTSB report.
The left engine eventually crashed to the ground, and a fire ignited near the left wing attachment, continuing until the plane crashed into a nearby warehouse and two buildings, the report said.

Investigators also said the left ft pylon broke, allowing the engine to separate, adding that they found small cracks that developed over time near the bottles in the mount, until they failed under normal pressure.
